·        The patient or his/her healthcare professional* decide that recreational therapy is needed.

·        The referral form gets completed by either the patient or the healthcare professional and also gets the treating physician to write a prescription for

“recreational therapy, eval and treat

for 2-3 times per week”.

·        Stella Husch, along with the other contractual CTRSs, will decide which therapist might be the best fit to administer the recreational therapy treatment.

·        The treating CTRS will contact the patient and/or the healthcare professional who referred him/ her about setting up an initial meeting at the patient’s home.

·        At the initial assessment meeting, the CTRS will asks a few questions of the patient (and/ or his family or health care professional if needed) in order to identify issues or concerns.  The initial assessment meeting

will allow the CTRS and the patient to agree on a treatment plan and goals.

·        The CTRS will set up a series of appointments for the TR sessions.

·        The CTRS and patient will determine progress and continue as planned.

·        The CTRS will then complete needed paperwork after each session and submit to Strive.  Strive will send in the paperwork to the patient’s auto insurance company and await payment.

·        The CTRS and patient will determine progress and continue as planned.  Various team meetings or prescription renewals might be needed from time to time.

·        The patient or his/her healthcare professional* decide that recreational therapy through group programs will be the best benefit.

·        The referral form and/ or the social group registration form gets completed by either the patient or the healthcare professional.

·        You will receive email notices from events@striveonline.org and sometimes phonecalls from the CTRS who will be supervising the social group outing.

·        If you decide you would like to attend an outing, you either contact the person in charge of the event (as indicated by the email or phonecall notices) and let them know you are interested.

·        You will either need to pay for the cost of the recreational/ social outing or you will be invoiced the amount due to Strive, Inc.

·        Limited transportation is sometimes provided at an additional cost of $10 per participant.

·        You’ll meet great people and have fun on whatever outing it is!  Enjoy!
